Who we are.

Elevate Health Rx is a trading name of Elevate Wellness Clinic, LLC, 3563 Robert Burns DR, Richfield, OH 44286. This policy explains what we do with information about you.

What we collect.

Information you give us, such as your name, contact details and the answers you provide in our eligibility check. Information about your orders, if you buy a program. Messages you send us. And standard technical information about how you use the site, such as your device type and the pages you visit.

Why we collect it.

To work out whether one of our programs may be appropriate for you. To connect you with a licensed provider. To deliver and support your program. To communicate with you about it. To meet our legal obligations.

Health information.

Some of what you tell us in the eligibility check is health information, and it is handled with more care than the rest. It is shared with the licensed provider network so a provider can review your file. It is not used to build advertising audiences, it is not sold, and it is not shared with advertising platforms.

Cookies and tracking technologies.

This site runs two advertising tools, and neither loads until you have made a choice about them.

The Meta Pixel, which measures our advertising. CallRail, which changes the phone number shown on the page so we can tell which advertisement a call came from, and which sets its own cookies to remember that during your visit.

On your first visit you will see a notice. Marketing cookies are off until you turn them on, essential ones keep the site working, and you can change your mind at any time from Manage tracking choices at the bottom of any page. If your browser sends a Global Privacy Control signal we keep marketing off and tell you we have done so. Your Privacy Choices is the one place to change this and everything else we do with information about you.

We do not place an advertising tag on the eligibility questionnaire and we send no advertising event from it. Your answers are never given to an advertising tool.

If we ever record a telephone call, we will tell you at the start of that call.

Who we share it with.

The licensed provider network that reviews you and prescribes, and the licensed pharmacies that dispense. The service providers who help us operate, including our customer relationship system and our payment processor. Professional advisers such as our lawyers and accountants, where they need it. Government or law enforcement, where the law requires it. And a successor, if the business is sold, who would be bound by this policy.

When you have accepted marketing cookies, Meta and CallRail also receive the technical information described above. Neither receives your eligibility answers.

What we do not do.

We do not sell your personal information. We do not share it for cross-context behavioral advertising. We do not use your health information for targeted advertising. If any of that ever changes, this page changes first.

Your choices.

You can ask what we hold about you, ask us to correct it, ask us to delete it, and opt out of marketing at any time. Email support@elevatehealthrx.com and we will respond within 45 days. We will not charge you for a request and we will not treat you differently for making one.

Text messages.

If you give us your mobile number and agree to be contacted, we may send you texts about your eligibility and your program. Consent is not a condition of purchase. Reply STOP to any message to opt out, or HELP for help. Message and data rates may apply. Full details are in our SMS Terms. We do not sell or share mobile phone numbers or SMS opt-in consent with third parties or affiliates for their own marketing purposes. Mobile information collected for text messaging is shared only with the service providers who deliver the messages on our behalf.

Security.

We use administrative, technical and physical safeguards to protect your information, including encryption in transit and at rest, access logging, and multi-factor authentication on accounts that can reach your record.

How long we keep it.

We keep order and billing records for as long as the law requires. We keep records of your consent for as long as we rely on it. We keep everything else for as long as we need it for the purposes above, and then we delete it.

Children.

Our services are for adults. We do not knowingly collect information from anyone under 18. If someone tells us in the eligibility check that they are under 18, we stop and we do not save what they entered.

How this relates to HIPAA.

This is a general privacy policy and it does not depend on HIPAA. We do not practice medicine, we are not a pharmacy, and we do not bill insurance, so whether HIPAA regulates us directly has not been determined. The providers who treat you and the pharmacy that dispenses your medication are covered entities in their own right, and information you give them directly is protected by HIPAA under their own notices. There is no such thing as HIPAA certification, and we do not claim it.

Changes.

If this policy changes, we will post the updated version here and change the date at the top.
