About these terms.

By using this site you agree to these terms. They are between you and Elevate Wellness Clinic, LLC, trading as Elevate Health Rx, 3563 Robert Burns DR, Richfield, OH 44286.

What Elevate Health Rx is.

Elevate Health Rx provides wellness programs, coaching and concierge services, and coordinates access to an independent licensed provider network and licensed compounding pharmacies. Elevate Health Rx does not itself practice medicine or dispense medication.

No medical advice.

Nothing on this site is medical advice. Nothing here creates a provider relationship. Only a licensed provider can determine whether a program or medication is appropriate for you.

The eligibility check is not a medical decision.

The questions we ask help us understand whether a program may be a fit and route you correctly. They are not a diagnosis and they are not a clinical assessment. A licensed provider reviews your full history before anything is prescribed, and that review is where eligibility is actually decided.

Eligibility.

You must be at least 18 years old to use our services.

Programs and payment.

Program pricing is shown before you enrol. Programs may include recurring charges, which are disclosed at checkout along with how to cancel.

Subscriptions and cancellation.

If your program bills on a recurring basis, we tell you the amount and the interval before you agree to it. You can cancel at any time from your account or by emailing support@elevatehealthrx.com. Cancelling stops future charges. It does not refund a charge that has already been taken, except where the Right-Fit Promise applies.

The Right-Fit Promise.

If a licensed provider does not clear you for a program, you receive a full refund of what you paid, including the consultation, which is not charged separately.

Compounded medications.

Compounded semaglutide and tirzepatide are prepared by state-licensed compounding pharmacies. They are distinct, custom-formulated preparations. They are not independently FDA-approved, and they are not the same as the FDA-approved brand medications Ozempic and Wegovy, or Mounjaro and Zepbound. Medication is dispensed only when prescribed by a licensed provider through the TelegraMD network.

Your responsibilities.

Give accurate information. Inaccurate health information can lead to an inappropriate recommendation. Use the site and the programs only for yourself, and follow the instructions a provider gives you.

Third parties.

Your care involves independent third parties, including the licensed provider network and the dispensing pharmacy. They are responsible for their own services and are governed by their own terms and their own professional obligations.

Intellectual property.

The content on this site belongs to Elevate Health Rx. You may read it and share it. You may not copy it for commercial use without permission.

No warranties.

The site is provided as it is. We work to keep it accurate and available, and we do not promise that it will always be either. Outcomes vary between people and nothing here is a promise of a particular result.

Governing law.

These terms are governed by the laws of the State of Ohio.

Text messages.

If you provide your mobile number and agree to be contacted, Elevate Health Rx may send you text messages including appointment and program reminders, account and billing notifications, eligibility updates, and, if you separately opt in, promotional offers. Message frequency varies. To stop receiving messages, reply STOP to any text message. Reply HELP for help. Message and data rates may apply. Check with your carrier for details. Carriers are not liable for delayed or undelivered messages. Full details are in our SMS Terms.

Changes.

If these terms change, we will post the updated version here and change the date at the top.
